The Visiting Nurse Association Hospice of Northwest Indiana and the Center for Hospice Care announced a strategic affiliation to strengthen care in Northern Indiana.
The nonprofit organizations agreed to the legally binding framework to create a care network available to their patients in the North Central and Northwest Indiana regions.
“Excellent patient care is, and always will be, our top priority,” said John Mastrojohn III, president and CEO of CHC, in a press release. “This affiliation allows us to build on the strong legacy of both organizations while expanding access to compassionate, expert care. Patients and families can be confident they will continue to receive the same high-quality, patient-centered support they trust, now with the added strength of a broader regional network.”
A member substitution agreement will allow a seamless transition.
“We couldn’t have found a better partner to help us provide care to those with serious, advanced illness in the communities we serve,” said Bob Franko, VNA Hospice NWI president and CEO. “We share common values centered on providing patients and their loved ones with expert, compassionate care.”
The Center for Hospice Care, which was founded in 1980, offers hospice care, palliative care, grief counseling and community education. It serves Elkhart, Fulton, Kosciusko, LaGrange, La Porte, Marshall, Porter, St. Joseph and Starke counties.
The VNA Hospice of NWI, a pioneer of home hospice in the 1980s, has offices in Valparaiso and Crown Point. It provides hospice and palliative care, VNA Meals on Wheels of Porter County, the Medical Guardian help button, the Phoenix Center for Grief, and a partner of the national We Honor Veterans Program. It serves Lake, La Porte, Jasper, Newton, Porter and Starke counties.
